It'll show up as a question mark if it cannot find the custom image or only the shop icon is set, not the postbit icon. There's two dropdowns to choose images from in the item manager.

I'm unable to confirm this issue, please ensure you've reverted any customised templates for vBShop.

If you are talking about something like this:

It seems that may be a item that was deleted at one time or another when users owned it, here is the inspection of that ? image.

Code:
<img alt="" src="http://www.**********.net/dbtech/vbshop/images/unknown.gif">
See where it says alt="" their should be a description in there what the item is for, like this:

Code:
<img alt="Donation to Forum - $1" src="http://www.**********.net/dbtech/vbshop/images/gifts/ribbon (2).gif" title="Donation to Forum - $1">

And how am I suppose to add the alt?

No, if it has no alt then it's a broken item. Try re-downloading and re-uploading, then manually execute /admincp/vbshop.php?do=finalise&version=111 and let it run till the end.

That should sort it :)
