Bug Double PM on Reply

I noticed this today on this site (DBT) when Dylan replied to one of my PM's. Then it happened on my site just a minute ago.
When PM's are replied to there are two sent.
Send me a PM and I will reply to it.

We just tested on here and were not able to reproduce the reported issue. Without a way to 100% reproduce it, there is no way to see what the cause may be.

Just happened again Ozzy. I have not opened the PM's in case there is some way you want to troubleshoot.

Nah you can open it, I don't have a clue what might cause it at all, and like I said, without being able to reproduce it on demand, I don't know what to look for.
