Legacy Autofill for the title

I'd like to be able to autofill the title with something like this:

Application for {username}

Basically filling in username with the persons username that is filling out the form.

These three features are best reserved for a dedicated update that moves the form stuff in place of the first post of a thread, to be offered as an alternative to the current "above post list" display.

I'll flag them as Accepted, as I do want to release such an update, though I'll need more time than I currently have with our current release schedule.
