Approved Address Custom Fields

Not every country has the standard of USA for addresses.

Please add custom fields feature for addresses, so that we can add different fields with different fields types to get enough info about shipping and billing.

This feature is very important as we use the address for tax purposes and shipping purposes and we need to make sure we get the info we need.

Notes to self:

Defined globally in the AdminCP
Must be impossible to delete used fields - return an error message when attempting to delete fields that have been used in an address. Find out what happens when new fields are added, does the "field value" table receive empty rows?
Redesign sitewide and business profile address choosers.
Keep "Country" as a mandatory field, separate from custom address fields
