Bug Every Category uses the same two extension groups

Guess who? Another day, another glaring problem to fix. Now, for some reason, I've just noticed that despite having different extensions for different categories in the settings, EVERY category is only allowing two extension groups, regardless of the settings. I have one for Images and one for Zips and Rars, but despite not having Zips and Rars enabled for most categories everything only allows those two groups.

Can you be more specific as to where the permissions check fails?

Does it display the correct extensions in the table on the Modify page?

Does it produce an error message when uploading?

Does it produce an error message when saving a previously uploaded extension that is now incorrectly disallowed?

On every category it loads as if the extension groups "Images" and "Zips and Rars" were set, even though they are not. You cannot upload any other file type, you can upload zips and rars even when you aren't supposed to. When you go to upload or edit a file it lists the allowed file types as the various jpgs and png and zip and rar, and will actually let you upload those, even though those are not what are supposed to be allowed within the category's settings.
