Please try looking in your browser's JavaScript console for any JS errors, try using a completely default (unmodified) skin, as well as disabling all other add-ons.

There's no errors in that log pertaining to this add-on, so unfortunately I won't be able to assist further. vBulletin 4 and 5 is no longer supported. I recommend upgrading your forum to XenForo for a more modern experience.
