mysql error when writing reply?

hi

when i reply i get this

                FROM pt_issuenote
                WHERE userid = 1 && isfirstnote = '0';

MySQL Error   : Unknown column 'quiz_id' in 'field list'
Error Number  : 1054

and this another user

how to solve that plz?

note : i disable the hack is that affect or not?

@Belazor hi sir i dont install vbquiz

i install only vbactivity 3.0.0 (downloaded from vbulletin.org) + project addon of the vbactivity

so plz see it
